ScreenCam Multimedia Patient Record
Brief History:
Patient is about 70 yo male came to emergency room with acute gi bleeding and some dizziness in 1997. Patient has diverticulosis and had been seen previously in 1992 for similar acute gi bleeding, which had stopped spontaneously. On this second visit, surgery was required to stop bleeding.
Script of ScreenCam:
=> CPRS and Imaging have been launched and are seen on the screen (Note: unfortunately, the cursor arrow doesn't show in this ScreenCam)
=> Patient has been selected - (these are all fake names)
Notice image abstracts (thumbnails) loaded automatically for this patient (they serve as a menu for image selection)

=> Next we check medications and orders
=> Then user check Notes
=> Next check labs - showing worksheet with graph view
* On HCT graph, there are two acute drops in HCT, one at time of acute GI bleed in late 1992 and one during second GI bleeding episode in middle of 1997
=> User clicked on the lower GI colonoscopy image abstract (1992 bleeding episode) - group window appears showing images in that study (1992).
=> User clicks on left abstract - you can see the diverticuli in the colon.
=> User clicks on right hand abstract in group - now you can see the bright red blood in the colon (and so can any of the treating physicians)
=> Next, user looks at a bleeding scan that was done to try to locate the site of the bleeding - they couldn't find it on the film - but you can see it when you manipulate the window / level (contrast and brightness) of the image - see right hand side (box will indicate it) then screen is reverse (black <-> white)
=> Now user looks at the reports for this patient
=> User selected Imaging to see Radiology / Nuc Med reports
=> User selected report from Jul 27 1997 - we see the report and the group image window comes up automatically on the right side of the screen - this shows the images associated with the report -- this is a Nuc Med bleeding study. The user reads the report (see it scroll down) then views the image (changes window/level values).
=> Now user selects another study. The associated images (6 of them) come up in the group window on the right. User selects the first image in the study for viewing. Then user pages through the images by clicking on the "Image #" button in the upper right of the toolbar.
=> Now the user checks the patients notes. This is a cardiology note after the second bleeding event. Mentions possible AV anomalie.
=> Now the user checks the other note. This has a scanned advance directive attached, which is clicked on and viewed.
=> Now the user goes back to the Imaging Reports and selects one that has a scanned outside report attached to it.
=> Now the user clicks on the EKG button. An EKG is displayed for the patient. It has the full diagnostic resolution and can be zoomed (user clicks to do this).
